I just received the most random package in the mail. It was addressed from the Substance Abuse and Mental Health Services Administration in Maryland. What the heck, I’ve never even heard of them let alone know why I was mailed this:
Its not as if the address was mislabeled, my name is clearly written on it. Have any of you had something similar happen?

Free crap in the mail with a message on it? Often. Usually it’s less useful, like a set of address labels or a sticker. Usually the message to you, the recipient, is “send money.”
There was no note or anything from the association, but there was an order receipt inside saying I ordered it for a price of $0.00
one time i was on a website that had a free sample product that i just signed up for cause it was free and it was supposed to come in like 2 to 4 weeks, and i got somthin in the mail like 3 months later not remembering or expecting it, but i eventually figured it out.

I once worked for a volunteer organisation and got something simular to that. I haven’t opened it yet. It’s just been sitting there, maybe i should open it. 
Ahah, believe it or not, the thought of anthrax did enter my mind. I would sure hope the substance abuse administration wouldn’t sent me anthrax.
Maybe you have an online friend who “purchased” it on your behalf.
